So, now that you have booked the assessment there are three questionnaires for you to complete – The Family Questionnaire, The Eyesight Questionnaire and the Young Person’s Questionnaire. These are all linked below.

There is also the School’s questionnaire which you will need to email the link of to your chosen member of the school, college, or university which your child attends along with a statement saying that you give them permission for you to share the requested information with me.  

If your child is home schooled please complete this questionnaire yourself.  Whilst there may be some questions which aren’t relevant to your circumstances just complete it to the best of your ability.

Once you have completed and submitted the appropriate questionnaires, head to the ‘Preparing for the Assessment Section so that you and your child are ready for when the assessment day comes.

School Questionnaire

Please pass this link to a member of staff at your child’s school/college or university who knows your child well e.g. the class teacher, SENCO or Head teacher. If your child is homeschooled, you may complete this form yourself (although not all questions will be relevant).

I have recently asked Zoë Brown at Mancroft Learning, a dyslexia specialist in South Norfolk, to assess my son/daughter, CHILD’s NAME, for dyslexia.  
 
I would be very grateful if you could spare some time to complete the following questionnaire on CHILD’s NAME’s educational development.  I consent to you giving Zoë this information about my child.  This also gives you an opportunity to outline your own thoughts about his/her recent progress. The information you provide will be written up in the completed diagnostic report and provide a valuable insight into CHILD’s NAME’s strengths and current areas of difficulties.
